The clinic will be held in the Foster Elementary School Gym located at 5931 School Street in Oak Forest. Appointments begin at 10:00am and a parent or guardian must accompany all individuals who will receive the shot.
In addition to a completed consent form which is attached to this email, all registrants will be required to provide their insurance card or Medicare ID, and parent/guardian driver’s license, or state ID. Individuals who do not have insurance coverage still qualify to receive the vaccine.
Pfizer is a two-part vaccine, with the second dose scheduled at Foster Elementary School Gym for Monday, December 13 starting at 3:30pm. Parents should ONLY sign up if you are able to make both dose 1 and dose 2 dates. For all patients aged 5-11, a parent, guardian, or non-custodial adult must be present and witness the vaccination.

The vaccination clinic is entirely voluntary. The vaccine is not a requirement for students to attend Forest Ridge School District 142, and the District does not hold any responsibility for the clinic.
